Phyllis Jean Fornoff

Phyllis Jean Fornoff, 65, of Manito, died at 11:20 a.m., on Sunday, October 24 at Memorial Medical Center, in Springfield. She was born May 10, 1956, in Pekin to Melvin and Alta May (Messman) Fornoff. They preceded her in death. She is survived by her sister, Marilyn (Steven) Stout, of Topeka, and her brother, Kenneth (Staci) Fornoff, of Bloomington. She is also survived by her nieces and nephews, Sara, Matthew, Lisa, Teresa, Ehron, Kristina and Tricia, and her great nieces and nephews. Phyllis was a graduate of Forman High School and Western Illinois University and retired from the Illinois Department of Public Health and Vital Records. She was a faithful member of Trinity Lutheran Church, an avid Cubs fan, an animal lover, a volunteer at TAPS and a loving and beloved sister and aunt. Visitation will be held at Maas-Hurley Funeral Home, Manito, on Thursday, October 28 from 5:00 until 7:00 p.m. Funeral services will be held at Trinity Lutheran Church, Manito, on Friday, October 29 at 10:30 a.m. Rev. Matthew Berndt will officiate. Burial will follow at Meadowlawn Cemetery in Manito. Memorial contributions may be made to Trinity Lutheran Church, or TAPS. Online memorial messages may be left for the family at Maas-Hurley Funeral Home in Manito is in charge of arrangements.
